Introduction to dermal fillers
Dermal filler injections have become increasingly popular due to the instant gratification results they provide are the fact that they are non-invasive. Dermal fillers help smooth out folds and wrinkles, add volume to the lips, and contour facial features that have lost their fullness due to aging, sun exposure, illness, etc.
The most common ingredient of dermal fillers are “Hyaluronic Acid”. In the United States the FDA has approved many different brands of dermal fillers namely; Juvederm, Restylane, Radiesse, Versa, and RHA collection of fillers. Botox and Dysport are not dermal fillers but rather neurotoxin that help reduce facial wrinkles such as the forehead and frown lines.
WHERE CAN DERMAL FILLERS BE INJECTED in the face?
Dermal fillers can be injected in the lips, cheeks, chin, jawline, temples, nasolabial folds (smile lines), under eyes, and marionettes. Lip dermal fillers are one of the most popular areas to get injections for hydration, plumpness (volume) and shape definition.
Cheek Filler injections are also highly requested due to the fact that they add volume and provide a youthful appearance. As we
age, we lose bone, fat and collagen leading to droopy and sunken cheeks.
Cheek filler is the fountain of youth as it helps lift and contour the face. Cheek fillers are made of hyaluronic acid, a gel-like substance that replaces lost collagen as a result of again, excessive sunlight exposure, genetics or simply lifestyle changes such as sleeping on one side of the face causing cheek asymmetry. Injecting cheek filler helps correct mild to moderate smile lines (nasolabial folds) that are starting to create a fold, like parentheses around the mouth area. This area of the face is an important area to address as it sculpts the cheek bones in a natural manner and provides a define side profile.
Jawline contouring has also become a popular trend amongst both the younger population and the older generation. With aging and lifestyle modification (smoking, sun bathing, fad diets leading to rapid weight loss) the facial fat and bone degenerate leading to a triangular facial shape. As a result, and over time, as the cheek volume is depleted, the skin gets more lax leading to “jowliness”. Dermal filler is able to restore the natural facial shape you once had and contour the jawline bone for a more defined and youthful facial balance and structure. Depending on the desired aesthetic goal, 2-4 syringes may be required for this procedure. Sometimes Botox is used in combination to jawline dermal filler to give a slimmer jawline appearance.
Fillers come in different viscosities (firmness) and based on the facial area being treated, a softer or a firmer dermal filler will be selected. The chin plays an important aesthetic role in the face by enhancing the size or projection of the side profile. Based on the ultimate goal, 1-2 syringes may be required for this procedure. Oftentimes, the jawline and chin contouring go hand in hand.
Using dermal filler for the tear troughs (under eye) provides a rested appearance due to filling in the sunken under eye groove. For safety, under-eye fillers are now performed with a cannula. A cannula is not a needle but rather a thin tube that delivers filler without puncturing through the vessels and arteries, hence a safe alternative to the old-school way of injecting under eyes with needles. This also prevents bruising and reduces social downtime. One of the commonly asked questions asked prior to determining if someone is a candidate for under-eye fillers is: Do you feel that you look tired even when you have had enough sleep?. It is important to ensure that the patient is not a surgical candidate (blepharoplasty) prior to injecting them with dermal filler to avoid suboptimal results. Hyaluronic acid dermal filler can correct the indentation caused by collagen loss under your eyes to give you a more rested and youthful appearance, provided there is not too much loose under eyes skin or puffiness. Dermal fillers do not treat dark circles (pigmentation) but rather hollowness.
Another common area to inject dermal fillers are the nasolabial folds (smile lines or parentheses). Nasolabial folds are the indentation lines on either side of the mouth. Oftentimes, cheek filler will soften the appearance of these smile lines by lifting the cheekbones. Based on how mild or severe these smile lines are, dermal fillers can be injected to help restore facial volume and by reducing the folds that give the
illusion of an aging face. The lines that extend below the nasolabial folds are the marionettes. Marionette lines get their name from marionette string puppets. Marionette lines are wrinkles that start developing as early as 20s but typically in the 40s. They create sagging in the lower half of the face, as the skin in that area is thinner. Dermal fillers can correct this area of the face in a natural manner when the appropriate product has been selected for the patient.
How Long Do Lip Fillers Last?
This is a very common question. There are many factors that impact the longevity of dermal fillers. Typically, dermal fillers last anywhere from 6-12 months based on your age, gender, lifestyle and metabolism. Hyaluronic acid is the most common dermal filler used which is a natural compound that aids in the production of collagen. The more you sweat by going to the gym, sauna, pool, beach etc. the faster your body metabolizes the filler at a faster rate. I recommend getting fillers twice a year (every 6 months) so that the filler is not completely depleted for a fresh start every time. There are some fillers that are not made of hyaluronic acid such as Radiesse that last a little longer, however, Radiesse cannot be dissolved as in the case of hyaluronic acid. Hyaluronic Acid is a substance that the body naturally produces. When we are younger and in our 20’s and early 30’s, we have more collagen compared to when we are in our mid 30’s and older. The youthful appearance that we have in our younger years is due to the natural hyaluronic acid in our faces and bodies. Dermal fillers containing hyaluronic acid are an excellent non-surgical alternative to plastic surgery. Nowadays, more and more people opt for getting non-invasive procedures with dermal fillers and other anti-aging options at a Med spa rather than getting plastic surgery. The main reason for choosing the non-invasive route is because plastic surgery is non-reversible, leaves a scar (give away), and the worse case scenario is if the final results are unfavorable and now there is no going back.
